Please use this form to make your formal request for up to $1,500 for professional development through the UW Oshkosh Whitburn Center. Applicants must include: 1) Their contact information 2) A copy of their resume verifying completion of an MPA program at an accredited university 3) A brief statement of need for the professional development funding 4) A detailed outline of costs, including all expenses (such as travel costs, registration, etc.)
