Outstanding Performance Award (OPA)

Nomination Form 

PURPOSE
The University of Wisconsin Oshkosh (UWO) recognizes the contributions of university staff, limited-term employees and project appointment employees by selecting individuals for an Outstanding Performance Award. The Award recognizes excellence in providing quality service in one’s area of assignment and the University community and demonstrates the importance to the academic environment and overall reputation of the University.

ELIGIBILITY

  • Current university staff, limited-term employees and project appointment employees with at least .50% FTE and have worked at UWO for at least three years.
  • Never received the award.

SELECTION PROCESS 
Nominations – In the spring of each year, nominations for the Outstanding Performance Award are solicited, but may be received at any time.

Selection Committee – The Awards and Recognition Committee as appointed by the University Staff Advisory Council, who makes award recommendations to the Chancellor.

Chancellor – determines the recipient(s) of the Award.

PROCEDURE

    1. Nominations are submitted to the University Staff Awards and Recognition Committee. All University personnel and students are encouraged to submit nominations. Self-nominations will be accepted. See eligibility requirements above.
    2. The Committee verifies the employment of nominees. All nominees are notified regarding eligibility.
    3. The nominee submits the required documentation to the Committee for review.
      1. Two letters of support by individuals other than the nominator.
      2. A brief statement describing what it means to you to be a UWO employee and how you support the reputation of the University.
      3. A list of your participation in University and Community activities.
    4. The Committee submits to the Chancellor the names and accompanying documentation of up to four (4) and no less than two (2) individuals (unranked) they recommend for the Award.
    5. The Chancellor selects the recipient(s) of the Award.

AWARD

  • Public University recognition.
  • A framed certificate signed by the Chancellor and Provost.
  • A picture of the award recipient(s) will be placed on the University Staff Senate web site.

PRESENTATION

The University Staff Outstanding Performance Award(s) are presented during the University’s annual Opening Day Awards Ceremony in the fall.
