The sports medicine staff at the University of Wisconsin Oshkosh look forward to providing services to student-athletes returning for the upcoming school year.
Returning student-athletes for 2025-2026 will need to access ATS to update and complete necessary information. NO annual physical update exam is required this year, though we highly recommend student-athletes have an annual physical exam. The Physical Exam Form can be found below. A video to the right explains everything that needs to be completed prior to any team activity. Listed below is the necessary information that needs to completed:
1. ATS Information. Please access your ATS medical account and review information for each tab of “Athlete Information”. Please verify that information is correct and add any new information. Make sure to verify your health insurance information and that a picture of both the front and back of your health insurance card is uploaded. Access to the ATS website is below. Contact your teams Licensed Athletic Trainer with any questions.
2. FORMS tab in ATS. Under the “Forms” tab of athlete information in ATS, please complete the following forms:
a. Returning Athlete ONLY Questionnaire
b. Forewarning
c. Health Authorization
d. Insurance Verification
e. NCAA Athlete Education
f. Helmet Warning (For Football Athletes Only)
