This is a two-step process for approval to film on-campus.

Step one: Prior to filling out this form, please reserve your rooms through the proper UWO channels for room reservations. If your location is an area that is owned by a department, you must receive prior approval to use that space from that department e.g. (Music Hall Lobby, X Theater, library, etc.) For help with spaces on campus, contact your instructor or student org advisor. More information, including contacts in other departments, can be found in the RTF Production Hub.

Step two: Your approval request will be directed to the UWO Police and the Office of Compliance, Risk and Integrity for risk management assessment, to ensure there are not any unforeseen or potential safety concerns. This approval request does not guarantee your location.

ALL REQUESTS SHOULD BE MADE TWO WEEKS IN ADVANCE OF YOUR SHOOT.

Identify any action taking place during your campus location scenes that might be misconstrued by the public, such as fighting, yelling, brandishing weapons, etc. (Guns may not be used on any exterior campus location.)

Be as specific as possible. Access will not be granted to the following areas: rooftops, heating plant, Counseling Center, Health Services and Nursing Ed.
