Transfer Credit Re-Evaluation Request

If you believe that there has been an error or oversight in your credit evaluation, please contact the Registrar’s Office for assistance (TCHelp@uwosh.edu).  In the event of an unresolved disagreement with the evaluation, you will be referred to the appropriate approving authority for a second review.  You should be prepared to supply a course syllabus or a letter of description from the previous college if you wish to appeal the initial evaluation. Requests for a change of evaluation should be sent to TCHelp@uwosh.edu. Please provide the following information in the email. The items marked with an asterisk* are REQUIRED:

* Student First and Last Name
* Student ID number
* External Institution (Sending Institution)
* External Course Subject and Number
* Current Evaluation for External Course
– Requested UW Oshkosh equivalent Course Subject and Number. Note: this is not required but it is helpful to expedite the approval process
– Approval (if you have it) from Approving Authority. Please attach the email showing approval and date. Note: if approval is not included, we will route request to appropriate approver.

We will include you in our email to the approver and ask that they Reply All so you can see the response as soon as possible. We will follow up once we have made the change in the student record and in our system/Transferology.

* Student First and Last Name: Tina Titan
* Student ID number: 0123456
* External Institution: Concordia University
* External Course Subject and Number: ED 1230 Nature of the Exceptional Child
* Current Evaluation for External Course: SPEC ED 1 Special Ed Elective
– Requested UW Oshkosh equivalent Course Subject and Number: SPEC ED 252 Students with Disabilities in General Education 
– Approval from Approving Authority: Email attached
