Advisory Board Agreement and Renewal Form

For new appointees and 3-year renewal for returning members.

  • 1. By January of their third year, a renewal letter will be sent to Members. If a Member chooses to renew for another three-year term, they need to notify the co-chair by February. This allows the co-chairs to determine if the campus needs to be contacted for applications. 2. Members are allowed to serve an unlimited number of terms, as long as they are meeting the expectations of Advisory Board Members. 3. If a Member needs to resign from the board, a written notification should be provided to the Program director.
