Registration
Head Start is ALWAYS recruiting eligible children for the current or upcoming school year! If you or someone you know has a child who will be 3 or 4 by September 1st, please ask them to call (920) 424-2166 for more information on how to apply.
What to bring at Registration
The following items are needed to COMPLETE your child’s Head Start application. You can still come to register even if you don’t have all the necessary paperwork, but it will be incomplete until we receive all the necessary paperwork.
Foster parents-If you are currently a Foster parent for the Head Start child, bring income reimbursement verification and any legal documents regarding custody arrangements.
Homeless families-If you consider your child to be homeless by the McKinney Vento definition, let the staff person know at time of application and bring a letter from the shelter or treatment facility (if you reside there)
HEAD START APPLICATION MATERIALS
- TANF /PUBLIC ASSISTANCE QUALIFIERS
- *Supplemental Security Income –SSI. Bring a recent letter from SSI with the amount received for that month.
- *Wisconsin Works-W2. Bring a recent month printout of W2 cash payments from your case worker.
- *Kinship- bring a recent month printout of Kinship money you received. Also bring legal documents regarding custody arrangements.
If you do not receive any of the above, bring any/all of the documents listed below:
- All W-2’s from 2019 OR Income taxes form 1040 from 2019 (preferred)
- Unemployment statements or payment history,(2019) or 1099 Miscellaneous Income (W-2)
- Child support statement printout for all children in the home for all 2019 (if any was received).
- Social Security Disability Income (any family members) – (1099 miscellaneous income (W-2), or payment history, or check stubs, or bank deposit history showing social security disability
- Social Security Award letter (any family members) – (1099 miscellaneous income (W-2), or payment history, or check stubs, or bank deposit history showing social security disability
- School grants/scholarships received in 2019 – 1099 (W-2) with tuition information – award letter
- Proof of any other family income
Items below are suggested but not mandatory to complete an application for Head Start. Bring if they are applicable and /or available:
- Your child’s official birth certificate (preferred), crib card, baptismal record, or immunization record.
- A copy of your child’s most recent IEP or IFSP – applies if your child works/worked with area school district or B-3 for any special services.
- A copy of any legal custody/ placement paperwork if applicable.

Income Verification
Family income must be verified 12 months prior to enrollment. Applicants must submit either 12 months of check stubs prior to the date of application or the calendar year prior to the application, which would be W-2 or tax forms. We also need to verify child support, unemployment, SSI, W2 benefits or any other income coming into the home. Categorically eligible families where the enrolling child is a foster child or the child is homeless or Public Assistance recipients of SSI, Kinship or W2 are eligible with proper verification as well.
