Request Accommodations
The Center for Accessibility & Disability Resources (CADR) coordinates access and equity for students with documented disabilities at the University of Wisconsin–Oshkosh. We are committed to ensuring students have meaningful opportunities to fully engage in the educational environment through an interactive process. CADR staff provide student‑centered support that recognizes the uniqueness of each student. We strive to make the accommodation process clear and easy to navigate, while partnering with students throughout the process.

We encourage all prospective students to begin the accommodation request process as early as possible. Early submission is especially important for housing and other non‑academic accommodations, as these often require additional coordination and may have limited availability.

The Interactive Process for ADA Accommodations
The steps below outline the interactive process, starting with CADR working with students to determine accommodations and expanding to include instructors and campus partners to support access.
Step 1: Student Requests Accommodations
Students begin by identifying a need for disability‑related support and submitting an Accommodation Request form in AIM.
Step 2: Student Meets with a CADR Advisor & Provides Documentation
Students schedule an initial meeting with a CADR Advisor to discuss how their disability may impact access to academic activities. Relevant documentation (such as medical or psychological records, or an IEP if applicable) is submitted and reviewed in accordance with documentation guidelines. The documentation should meet UW Oshkosh documentation criteria.
Step 3: CADR Determines Eligibility for Accommodation
CADR reviews the documentation and information shared during the meeting to determine eligibility and appropriate accommodations. If eligible, CADR works with the student to develop an individualized accommodation plan and notifies relevant faculty members about approved accommodations through AIM. Student privacy is maintained by only sharing necessary information about the approved accommodations.
Step 4:Student & Faculty Implementation Meeting
This meeting is a collaborative conversation where students have the opportunity to share accommodation implementation strategies that have worked well for them in the past, and instructors can share methods that have been effective in their course. Students are not required to discuss any specific diagnosis or disability‑related information, as implementation decisions are guided by individual needs and course design.
Step 5: Periodic Review & Renewal
Students are encouraged to stay in communication with CADR and their instructors. Accommodations may be reviewed or adjusted as needs change. Additional or supplemental accommodation requests can be submitted through AIM.
Documentation Guidelines
The Center for Accessibility & Disability Resources follows guidance from the Association of Higher Education & Disability (AHEAD) when reviewing documentation for accommodation requests. Documentation helps CADR understand how a disability may impact access and is used to determine reasonable accommodations.
UW Oshkosh Documentation Guidelines:
Documentation should:
- Be recent, relevant and comprehensive, and contain test scores and interpretation if appropriate (e.g. learning disability reports, audiograms, ADHD, etc.)
- Demonstrate a substantial impact on one or more major life activities.
- Indicate whether the impact is current and stable or fluctuating (conditions that fluctuate over time may require more recent documentation and ongoing updates of documentation).
- Clearly describe the nature and extent of the disability using current professional standards.
- Substantiate the need for the students’ requested accommodations.
- Be provided by a licensed clinical professional familiar with the student’s history and functional limitations. The clinical professional should not be a member of the student’s family.
- Be submitted on official letterhead.
- Include the date, signature, credentials, and licensing or certification information of the evaluator.
Students with multiple disabilities are encouraged to identify all diagnosed conditions, so CADR can make the most appropriate accommodation recommendations. If documentation is incomplete or does not sufficiently address access needs, the University has the discretion to require additional documentation. Any costs associated with obtaining documentation are the responsibility of the student.
