To request a survey, please first contact Linda Freed (Director, Office of Grants & Faculty Development), who determines whether or not your survey requires approval by UW Oshkosh's Institutional Review Board.

In many cases, no IRB review is required.

If IRB review is required, the process (as described on the IRB web page) may take up to two weeks during the academic year and possibly longer during the summer.
