The sports medicine and athletic training team at the University of Wisconsin Oshkosh is looking forward to providing services to student athletes back for the upcoming school year.

Returning athletes must complete the following forms prior to any participation, practice or event, in intercollegiate athletics:

  1. Physical update form completed by physician
  2. Health insurance form if it has changed in anyway from the previous year
  3. Photocopy of both the front and back the health insurance card if your insurance has changed

You will also need to access our medical website ATS, and update all your information and make sure it is correct.

As of fall 2012, student athletes using a banned stimulant drug to treat Attention Deficit Hyperactivity Disorder (ADHD) or Attention Deficit Disorder (ADD), need to fill out an ADHD Medical Exception Drug Form. Your physician must sign this form and attach supporting documentation. A sample ADHD Evaluation Form is also provided below.
