The sports medicine and athletic training team at the University of Wisconsin Oshkosh is looking forward to providing services to student athletes back for the upcoming school year.
Returning athletes must complete the following forms prior to any participation, practice or event, in intercollegiate athletics:
- Physical update form completed by physician
- Health insurance form if it has changed in anyway from the previous year
- Photocopy of both the front and back the health insurance card if your insurance has changed
You will also need to access our medical website ATS, and update all your information and make sure it is correct.
As of fall 2012, student athletes using a banned stimulant drug to treat Attention Deficit Hyperactivity Disorder (ADHD) or Attention Deficit Disorder (ADD), need to fill out an ADHD Medical Exception Drug Form. Your physician must sign this form and attach supporting documentation. A sample ADHD Evaluation Form is also provided below.

Annual physical updates are offered on campus twice per year: in October for winter and spring sport athletes and in April for fall sport athletes. Dates and times will be posted on our website. If you are unable to attend these sessions you may have your update completed at the Student Health Center or by your personal physician. This must be physically handed in.
Check to make sure everything is current on ATS medical website. If your insurance has changed we need you to update the changes on the ATS medical website. Also, physically hand in the health insurance form. A photocopy of both the front and back of the health insurance card is also to be handed in if your insurance has changed.
