ADHD/ADD Documentation

The NCAA requires students diagnosed with ADHD or ADD to have proper documentation prior to any athletic participation.

The NCAA requires all student athletes who have been diagnosed with Attention Deficit Hyperactivity Disorder (ADHD), Attention Deficit Disorder (ADD) and/or like conditions, and are currently prescribed banned stimulant drugs (ex. Ritalin, Stattera, Adderall, Concerta, etc.) for treatment, to have proper documentation handed in prior to any athletic participation.

Effective since August 2012, the NCAA now requires documentation of a comprehensive clinical evaluation to support treatment with NCAA banned stimulants.

The following will need to be on file with UW Oshkosh sports medicine prior to participation:

1) ADHD Medical Exception Drug Form

2) Supporting clinical evaluation notes from your physician who diagnosed you with ADHD or ADD

These forms along with a sample evaluation form for your physician can be found under student athlete information. We recommend starting this process early in order to have all forms complete prior your arrival at UW Oshkosh in fall.
