SCHEDULE OF FEES

Regular Fees (registrations received Sept. 1, 2009)
$400.00 full summit -- includes all summit sessions and materials, Monday-Tuesday-Wednesday breakfasts, Monday-Tuesday lunches, refreshments, and summit reception (sponsored by Children's Hospital of Wisconsin and featuring MadiSalsa)

$200 Monday-only -- includes all Monday sessions and materials; Monday breakfast and lunch, Monday refreshment breaks and Monday summit networking reception (sponsored by Children's Hospital of Wisconsin and Featuring MadiSalsa)

$175.00 Tuesday-only -- includes all Tuesday sessions and materials; Tuesday breakfast and lunch and Tuesday day refreshment breaks

$155.00 Wednesday only -- includes all Wednesday sessions and materials; Wednesday breakfast and refreshment break

$25.00 guest registration for Monday summit reception (reception fee is already included for registered summit participants)
