Requesting Your Official Transcripts:
The University of Wisconsin Oshkosh Records Office must receive a signed, written request in order to process your official transcripts. For your convenience, you can obtain our Transcript Request Form online.
- Download Transcript Request forms
- If you cannot download the form, we need the following information from you:
- Name
- Former names
- 7-digit campus ID# or Social Security # (we need this information to locate your record)
- Date of birth
- Mailing address
- Phone number
- Address to where you want your transcript mailed
- Payment
- Your signature

Payment for your request:
The cost is $5.00 per transcript for regular service (3-5 day processing) and $7.00 per transcript for rush service (24 hour processing). Payment must be received before the transcript request will be processed. Checks or money orders (made payable to UW Oshkosh), credit cards (Visa, MasterCard or American Express) with expiration date, and cash are accepted. Transcripts will not be issued to students who have any outstanding financial obligations to the University. Please call us for information about overnight delivery to the continental United States.
General Information:
The Records Office is located in the Registrar's Office, Dempsey 130. We accept transcript requests from 7:45 am - 4:00 pm , Monday-Friday. A photo ID is required when picking up transcripts.
An official transcript is a record of your entire academic coursework (both graduate and undergraduate). It is printed on security paper and bears the signature of the Registrar and the embossed school seal. Each transcript is mailed in an individually sealed envelope with the Registrar's signature across the seal. Faxed transcripts may not be considered official. It is up to the receiver of the transcript to decide if the faxed information can be used in place of an official transcript.
