Dates:
You must give us one date so we will know approximately when you
plan to administer the survey. If scheduling for a 14-week class, please
do not schedule the date of the survey to be administered until after
week 8 of the semester. We will mail the survey material to you approximately
one week before the date you give us. You may administer the survey
any time after receiving the material. return to filling

Data Retention:
Please store your
reports in a safe place. You may need them later for promotions or awards. Copies are available at a cost of 10cents per page starting with the Spring 2003 semester.
Handicapped
Students:
If you have a visually impaired student(s) in your class, or if a student(s)
has a dexterity or writing problem, please contact us to request a large
print/easy mark edition of the survey form.
Distribution:
Reports are mailed after grade lists have been submitted to the Registrar's
office, and all survey processing has been completed. This entire process
takes 3-4 weeks after the last day of class.

Confidentiality:
Electronic files of Student Survey data are maintained for the purpose
of constructing intra-department comparative tables. Files (SOS and Dept.)
do not contain faculty names. Since it has been determined that these
files are public records under state open records law, they are retained
for six years. |
Validity:
Analyses are based on the responses of the students present in the
classroom at the time of the survey. Students may not complete surveys
in the SOS Office. To obtain reliable student ratings, it is recommended
that data be collected from at least ten students and from at least two-thirds
of the class. A computerized survey report is not produced for less than
five students. If you have reason to think that the results of a survey
might be badly skewed for any reason, contact the Student Opinion Survey Office to reschedule. |
