Prior to departing the U.S., students who are finishing their academic program at UW Oshkosh must complete the following form.

Your SEVIS number is located in the upper left corner of your Form I-20.

Please list your new address. If you are not moving and going to continue to live in your same place of residence, leave blank.

Graduation Date

Expected Date of Depature

ISS will confirm this date once you have departed the U.S. through SEVIS.
