Application

To apply, please open and save the following application form to your computer. Fill out the form, and save the document with your last name and the date in the title, with the format LastName.MMDDYY.doc. For example, if Eric Kuennen were to submit his application on January 15, 2009, he would save the file as Kuennen.011509.doc. Then email the application document to excel@uwosh.edu.
