The registration deadline is Jan. 28, 2013. After this date, please call the Division of Lifelong Learning and Community at (920) 424-1129 or toll free at (800) 633-1442 to determine program availability. Early registration for this popular program is strongly recommended.
Fees for professionals
Complete Symposium: $219 (includes Thursday and Friday Symposium sessions, refreshments, materials, CD with Symposium handouts and Thursday lunch)
Thursday only: $179 (includes Thursday Symposium sessions, refreshments, materials, CD with all Symposium handouts and Thursday lunch)
Friday only: $159 (includes Friday Symposium sessions, refreshments, materials, and CD with all Symposium handouts)
Fees for UW Oshkosh and UW–La Crosse students
Complete Symposium: $35 (includes Thursday and Friday Symposium sessions, refreshments, materials, CD with all Symposium handouts and Thursday lunch)
Thursday only: $25 (includes Thursday Symposium sessions, refreshments, materials, CD with all Symposium handouts and Thursday lunch)
Friday only: $20 (includes Friday Symposium sessions, refreshments materials and CD with all Symposium handouts)
Credit for Symposium Attendance
CECHs: Category I CECHs will be available to Certified Health Education Specialists through UW–La Crosse (a National Multiple Event Provider). These CECHs require an additional fee:
- $16 – for Thursday, Feb. 7, 2013: 6 Category I CECHs
- $ 8 – for Friday, Feb. 8, 2013: 3 Category I CECHs
- $24 – for Thursday and Friday, Feb. 7–8, 2013: 9 Category I CECHs
For CECHs, please make out a separate check, payable to UW–La Crosse, and mail it with your Symposium registration form and fee to:

Continuing Education Units: CEUs are available for Symposium participants. CEUs will be awarded as follows: 0.6 CEUs for Thursday only; 0.3 CEUs for Friday only; and 0.9 CEUs for Thursday and Friday.
Lodging
Accommodations are available at reduced rates at The Madison Concourse Hotel. Please register online at http://concoursehotel.com, or call the facility directly at (800) 356-8293. Rates are $70 single, $110 double. Please indicate that you are attending the Adolescent Health Symposium when making your reservation. To receive these reduced rates, you must register no later than January 16, 2013. We urge you to make your sleeping reservation as soon as possible. You can cancel your sleeping room reservation with no penalty before 4 p.m. on Feb. 6, 2013.
Cancellation Policy
If UW Oshkosh Continuing Education receives notice of cancellation by the registration deadline (Jan. 26, 2011), you will receive a full refund of fees, minus a $20 processing charge. Substitutions for this program may be made at any time. Please inform us of such substitutions in advance, if possible. UW Oshkosh reserves the right to cancel any program due to insufficient enrollment, as well as the right to limit enrollment due to excessive demand.
