Living Healthy Community Clinic

We regret to inform you that the Living Healthy Community Clinic (LHCC) will end healthcare services Dec. 28, 2017.

 

During the transitional period, the director will work with LHCC clients to assist in finding appropriate primary healthcare service options within the community.  There are several options.

All three health systems in the area, which were strong supporters of LHCC, will accept new primary care patients:

  • Ascension/Affinity Medical Group. Call (920) 628-9551 or any specific clinic location to make an appointment.
  • Aurora Health Care. For assistance finding a healthcare provider, call 1-888-863-5502.
  • ThedaCare. Call (920) 237-5000 to make an appointment in Oshkosh or (920) 729-6088 to make an appointment in Neenah.

 

Partnership Community Health Center (PCHC) is a federally-qualified community health center that currently provides primary medical, dental and behavioral healthcare to low income and underserved populations.  The PCHC clinic is located at 2310 Westowne Ave. in Oshkosh. To make an appointment with PCHC, call (920) 731-7445.

 

The Oshkosh community is home to the St. Francis Community Free Walk-in Clinic that offers general medical care and some specialized medicine for the uninsured and underinsured. The St. Francis clinic is located at 1000 N. Koeller St. in Oshkosh. To make an appointment with St. Francis, call (920) 230-2273.

 

Patients who have utilized LHCC may obtain copies of their medical records or have a copy sent to a physician of choice. Written authorization is required to release medical records to another healthcare provider. Call (920) 424-1242 or email lhcc@uwosh.edu for more information. LHCC intends to retain patient care records through March 31, 2018. At that time LHCC will reevaluate and determine the status of remaining records. Wis. Stat. § 146.819 (2015-2016) will be followed for medical record disposition.
