Hate Crime & Bias Incident Report
This report is for any student, staff member, or faculty member at the University of Wisconsin Oshkosh that believes that they have been a victim of discrimination or harassment while on campus. Please fill out as much of the information as you can so that the incident can be dealt with appropriately. You do have the right to remain anonymous.
