AT Admission Portfolio

The Admission Portfolio for Athletic Training must include the following:
     *For help in creating your portfolio, follow these directions.

  1. A home page with a headshot/photo and a short biography, which includes a personal statement about your desire to enter the Athletic Training Education Program
  2. You will need to create one folder to contain all of your Admissions Materials. This should include the following items:
    • An admissions essay/letter addressed to the Athletic Training Admissions Committee, covering the following topics:
      • A little about your background and your experience with athletic training.
      • What sparked your interest in athletic training.
      • Who you would consider your role model or mentor and how they have influenced you in athletic training.
      • Some things that you learned about athletic training from your observation.
      • Where you see yourself after completing the program, including career, setting, further schooling, location.
    • A formal resume including the following sections (if appropriate):
      • Education section
      • Experience section
      • Honors and Awards
      • Activities and/or Volunteer Experiences 
      • *You may include any other resume sections you think contribute to your document.
    • Your mid-term grade report (scanned in) including all your courses from this semester with signatures.
    • A copy of your current STAR Report (copied into Microsoft Word and formatted) with the following information highlighted: 
      • Overall GPA  and  Grades in the classes listed below
        - Biological Concepts (26-105)
        - Human Anatomy (26-211) * if applicable
        - Orientation to Kinesiology (77-121)
        - Medical Terminology (77-170)
        - Prevention, Recognition & Treatment of Athletic Injuries (77-171) * if applicable
        - Applied Anatomy & Kinesiology (77-173) *if applicable
    • 1 Letter of Reference from a non-family member who can speak to your ability to succeed (ie. a professor, a former ATC, a coach, a high school teacher)
    • Summary document (2 pages) of an interview with one current athletic training student of your choice.
    • Copy of 1 ATC/Clinical Staff evaluation chosen for you.
    • Copy of 1 Athletic Training Student evaluation chosen by you. 

 
Students who wish to be considered for admittance must email access to their completed Admittance Portfolio to the Admission Committee members by 9:00 p.m. on the Sunday following Spring Break (April 1, 2012).  You must email your portfolio to the following group of people (PLEASE include all): sipesr@uwosh.edu, calvertk@uwosh.edu, peiterse@uwosh.edu, zuberbij@uwosh.edu, ,johnsenj@uwosh.edu, stierl@uwosh.edu
