Duplicate W-2 Requests |

| Form Instructions and Fee Information Requests for other years are submitted directly to the UW Processing Center in Madison and can take up to 3 weeks to receive. Other Year Duplicate Tax Statement Request Form (2006 and earlier)
Request for previous year is submitted directly to UW Oshkosh and can take up to 2 weeks to receive. Previous Year Duplicate Tax Statement Request Form (2007) Previous year requests are not accepted until Mid-February of the current year.
