Duplicate W-2 Requests |

| Duplicate requests for the previous year are submitted directly to UW Oshkosh and can take up to 2 weeks to receive. Previous Year Duplicate Tax Statement Request Form (2008) Previous year requests are not accepted until Mid-February of the current year and are at no cost to you.
