SPECTERA VISION and DENTAL INSURANCE FOR 2008 IMPORTANT INFORMATION
You are not currently insured in the State Group Health Insurance plan but are in an appointment in which you may be eligible. Therefore, we are unable to specifically identify your eligibility to enroll in the Dental and Spectera plans. Spectera is having an Open Enrollment from October 8 – November 16, 2007 You may enroll in the Spectera Vision Plan during this Open Enrollment period. You may elect to cover yourself, yourself and your spouse/domestic partner; yourself and one eligible dependent or yourself, your spouse/domestic partner and children. The Spectera Vision Plan provides partial payment to help you offset the costs of annual eye exams (which are also covered under Uniform Benefits of your health plan), frames, lenses and contact lenses. It also provides a discounted refractive eye surgery at select locations. To locate a Spectera provider near you, you can call Spectera's IVR line at 1-800-839-3242 or you can log on to www.spectera.com , click on “Members and Future Members” and enter your zip code. Note--there has been a change in the premiums. Spectera does not issue ID cards. When receiving services, everyone must provide the employee's social security number or person/employee id number and the employee's date of birth. The employee's person id can be found on the earnings statement. Coverage becomes effective on January 1, 2008 . Important Note: If you enroll in Spectera, you must remain in the plan for the entire 2008 calendar year. If you are currently insured and wish to cancel your Spectera coverage, you need to complete an application to terminate coverage and submit it to your payroll office by December 1, 2007. New! -- This fall, for the first time, materials from Spectera are being distributed to employees electronically. If you wish to enroll or add eligible dependents that were not previously enrolled: -- Complete the online fillable application at: http://www.bussvc.wisc.edu/ecbs/vis-application-form-Spectera.pdf --Indicate in Section II either your social security number or person/employee id as your subscriber number. --Deliver the completed application to the Human Resources Office in Dempsey 328 no later than 4:30 p.m. on November 16, 2007 . No action is necessary if: you are already covered under Spectera and do not want to change coverage to include dependents not previously enrolled or if you do not want to enroll at this time.
------------------------------------------------------------------------------------------------------------ DENTALBLUE INSURANCE FOR 2008 IMPORTANT INFORMATION
This information applies to unclassified faculty, academic staff and short-term academic staff appointees, non-represented classified employees (including LTEs covered by the retirement system), nonrepresented graduate assistants, fellows and scholars and employees in-training. To enroll you must be insured or eligible to enroll in the State of Wisconsin Health Insurance program. Note: there are identical dental plans offered to represented classified employees by the Office of Employment Relations.
DentalBlue is offering an Open Enrollment Period from October 8 – 26, 2008
During the Open Enrollment period you may elect to: --Enroll in DentalBlue for single coverage, two-person coverage or family. DentalBlue will mail you an ID card by January 1. --Change from single to family coverage or add eligible dependents not previously enrolled. DentalBlue will mail you a new ID card by January 1. --Change from one of the offered plans to a different plan. These changes become effective on January 1, 2008 . If you're not sure in which plan you're enrolled, check your earnings statements. New! This fall, for the first time, the DentalBlue informational and enrollment materials are being distributed to employees electronically. Simply click on http://www.uwsa.edu/hr/benefits/ins/dentblue.htm to review the Plan Summary of the offered plans. If you wish to review or obtain a paper copy of these materials, please come to the Human Resources Office in Dempsey 328. Be sure to review the Plan Summary and pay particular attention to the differences between the three plan options. --With the Dentacare HMO option , you must use the Dentacare HMO dental providers to receive benefits. You may change Dentacare centers twice per calendar year if you desire. The Dentacare HMO option does not include any waiting periods for coverage. --The Preferred PPO option allows you to see the dentist of your choice. However, your out-of-pocket costs will be lower if you select a dental provider who is affiliated with DentalBlue as you will receive a higher level of coverage. Note: There is a three-month waiting period for coverage of basic and major services. --The Supplemental Plan is specifically designed to wrap around your health plan's dental benefits. To be eligible for the Supplemental Plan, your health plan must provide routine diagnostic and preventative coverage. If you change health plans and your new health plan does not cover diagnostic and preventative dental services, you must submit an application to cancel your coverage or change to the Dentacare HMO or Preferred PPO. Note: There is a three-month waiting period for coverage of basic and major services. If you are currently a non-represented employee and you later become a represented employee you must change dental coverage.
