Fall Benefit Enrollment Opportunities

From October 6, 2008 through November 14, 2008

For the employee-pay-all "optional benefit plans":

· Anthem DentalBlue dental insurance

· OptumHealth vision insurance (formerly Spectera)

· EPIC Dental and Excess Major Medical insurance

· U.W. Employees, Inc life insurance

All elections and changes made during this period will be effective January 1, 2009

For the second year we are helping the UW System to be “green” by providing enrollment information to employees electronically.

Anthem DentalBlue Dental Insurance

All eligible employees may enroll for coverage, change plans within Anthem DentalBlue (e.g., change from the HMO to the Supplemental plan) and add eligible dependents during this enrollment period. A domestic partner (and his/her dependents) is eligible for coverage under this plan.

NOTE: If the Supplemental Plan is selected, the employee must carry basic dental benefits under a health plan within the State Group Health Insurance program. The employee's eligible dependents do NOT need to be covered under the State Group Health Insurance program in order to be eligible for the Supplemental Plan.

EPIC Dental and Excess Major Medical + EyeMed Vision Discount Plan

All eligible employees may enroll during this special enrollment period. EPIC rarely allows employees who have previously cancelled coverage under the plan to re-enroll so this is an unusual opportunity.

The EPIC Dental and Excess Medical plan, offered exclusively to State and University of Wisconsin employees, provides supplemental Dental, Excess Medical, and AD&D coverage. This supplemental insurance plan, which enhances your other base insurance plan(s), has been available to WRS participants for 45 years. The Excess Major Medical coverage provides medical benefits for out-of-pocket costs, such as co-pays and deductibles, when related to a surgery or inpatient confinement.

Benefit Enhancements Effective January 1, 2009

Dental Benefits: The benefit is being enhanced by lowering the annual dental deductible from $200 per person per year to $75. Benefits are paid at 50% and the annual maximum benefit will increase from $750 to $1000. There is no change to the orthodontic benefit for current members.

Note: Those who enroll during the special enrollment period will have reduced dental benefits for the first two years of coverage. Dental benefits will be limited to an annual maximum of $500 and $750 during the 1 st and 2 nd years respectively. In the third year of coverage, the annual maximum will be $1,000. In addition, the waiting period for orthodontic benefits is twenty-four months.

AD&D Benefits: The AD&D benefits are doubling for 2009 for a benefit of $10,000 for the employee. The spouse and dependent AD&D benefits will also double. (There is no change to the annuitants' AD&D benefit.)

EyeMed: The complimentary EyeMed vision discount program will continue in 2009.

OptumHealth Vision (formerly Spectera)

Eligible employees may enroll and/or add or remove dependents during this period. A domestic partner (and his/her dependents) is eligible for coverage under this plan.

As of August 1, 2008, Spectera began marketing its plan as OptumHealth Vision. This is a change in branding only. There is no change in the benefits, no change in the provider network, no disruption to service or benefits administration, and no change in the legal entity underwriting the plan .  You may continue to see references to Spectera, Inc. as the legal entity that administers your OptumHealth Vision plan.  If you have questions about the brand name change or your benefits, please call the OptumHealth customer service toll-free line at 1-800-638-3120 to speak with a representative.

OptumHealth Vision currently uses the employee's Social Security Number as the ID number under the plan, however, all subscribers will be assigned an alternate ID number for 2009. OptumHealth Vision previously did not issue ID cards but all subscribers will receive ID cards for 2009.

IMPORTANT: Once enrolled for a plan year, you must remain covered for the entire calendar year. If you want to cancel coverage, you must submit an application by December 1 st to cancel coverage for the coming year .

When calling the provider to make your appointment simply mention that you have OptumHealth Vision coverage. You will be asked to provide the member unique identification number and the patient's name and date of birth in order to receive services. 

UW Employees Inc Life Insurance

UW Employees Inc Life Insurance is a term life insurance policy. Any eligible employee can enroll for coverage during this period, regardless of health. This policy provides an inexpensive way to increase your overall life insurance coverage. There are no changes to the premium or the benefit levels for 2009.
