Dual Choice Open Enrollment

 

 

DENTALBLUE INSURANCE FOR 2008

IMPORTANT INFORMATION

This information applies to unclassified faculty, academic staff and short-term academic staff appointees, non-represented classified employees (including LTEs covered by the retirement system), nonrepresented graduate assistants , fellows and scholars and employees in-training. To enroll you must be insured or eligible to enroll in the State of Wisconsin Health Insurance program. Note: there are identical dental plans offered to represented classified employees by the Office of Employment Relations.

DentalBlue is offering an Open Enrollment Period from October 8 – 26, 2008

 

No action is necessary if you do not want to change plans or change coverage.

During the Open Enrollment period you may elect to:

-Enroll in DentalBlue for single coverage, two-person coverage or family. DentalBlue will mail you an ID card by January 1.

-Change from single to family coverage or add eligible dependents not previously enrolled. DentalBlue will mail you a new ID card by January 1.

-Change from one of the offered plans to a different plan.

These changes become effective on January 1, 2008 . If you're not sure in which plan you're enrolled, check your earnings statements.

New! This fall, for the first time, the DentalBlue informational and enrollment materials are being distributed to employees electronically. Simply click on http://www.uwsa.edu/hr/benefits/ins/dentblue.htm

to review the Plan Summary of the offered plans. If you wish to review a paper copy of these materials, please come into the Human Resources Office in Dempsey 328.

Be sure to review the Plan Summary and pay particular attention to the differences between the three plan options .

-With the Dentacare HMO option , you must use the Dentacare HMO dental providers to receive benefits. You may change Dentacare centers twice per calendar year if you desire. The Dentacare HMO option does not include any waiting periods for coverage.

-The Preferred PPO option allows you to see the dentist of your choice. However, your out-of-pocket costs will be lower if you select a dental provider who is affiliated with DentalBlue as you will receive a higher level of coverage. Note: There is a three-month waiting period for coverage of basic and major services.

-The Supplemental Plan is specifically designed to wrap around your health plan's dental benefits. To be eligible for the Supplemental Plan, your health plan must provide routine diagnostic and preventative coverage. If you change health plans and your new health plan does not cover diagnostic and preventative dental services, you must submit an application to cancel your coverage or change to the Dentacare HMO or Preferred PPO. Note: There is a three-month waiting period for coverage of basic and major services.

If you are currently a non-represented employee and you later become a represented employee you must change dental coverage. Contact your campus benefit office for procedures and forms.

--Deliver the completed application to the Human Resources Office in Dempsey 328 no later than 4:30 p.m. on October 26, 2007 .
