Wisconsin Medicaid Family Planning Only Services Program
Wisconsin women seeking contraception are now able to get confidential family planning health care services and supplies paid for through the Family Planning Only Services Program. The goal of Family Planning is to help women reduce unintended pregnancy, sexually transmitted diseases and obtain family planning health care services.
Who should consider this program?
- Women wanting contraceptive services, STI screening.
- Men wanting Sexually Transmitted Infections (STI) screening and treatment
What services are included?
- Birth control
- Routine reproductive health exams and tests
- Pregnancy testing and counseling
- Emergency contraception
- Education and information
- Urinary tract infections, STDs and vaginal infections will be covered
Who can get these services?
If you answer yes to the following questions there is a good chance you qualify for the Family Planning Only Services Program.
- I am a 15-44 years old.
- I am a U.S. citizen living in Wisconsin.
- I meet the income requirements.*
Monthly income limits (300% of federal poverty level):
- One-person household $2,707.50
- Two-person household $3,642.50
- Three-person household $4,577.50
Who can I see?
- UW Oshkosh Student Health Center
- Planned Parenthood
- Any health care provider or family planning clinic that accepts the Forward Health ID card.
To Enroll
Enrollment in the Family Planning Only Services Program may be completed through:
- Wisconsin Family Planning Only Services: 800-291-2002
- The Wisconsin Access website
- Online enrollment Instructions
- Go to the Access online website at access.wisconsin.gov and click "Apply for Benefits".
- Start a new application.
- Create an account with a user ID and Password you will remember.
- Log in.
- Apply for FAMILY PLANNING Waiver. Follow all of the instructions.
- NOTE: Use your local/Oshkosh home and mailing address.
- When you receive the benefit letter and Forward Health card, bring a copy to the Student Health Center to receive free family planning services.
RE-ENROLLMENT NOTICES WILL BE SENT DIRECTLY TO YOU BY EROLLMENT SERVICES.
