NOTE CHANGES IN THE FAMILY PLANNING WAIVER PROGRAM ENROLLMENT PROCESS:
Enrollment in the Family Planning Waiver Program is completed through the "Access" online website. Forms are also available at the Student Health Center.
- Go to the Access online website and click "Apply for Benefits".
- Start a new application.
- Create an account with a user ID and Password you will remember.
- Log in.
- Apply for FAMILY PLANNING SERVICES ONLY. Follow all of the instructions.
- WHEN YOU RECEIVE THE BENEFIT LETTER AND FORWARD HEALTH CARD, BRING A COPY TO THE STUDENT HEALTH CENTER TO RECEIVE FREE CONTRACEPTIVE SERVICES.
RE-ENROLLMENT WILL BE DIRECTLY THROUGH THE LOCAL DEPARTMENT OF HUMAN SERVICES OFFICE. Forms and instructions are available at the Student Health Center.
Birth control supplies will not be available every month as in the past. When picking up a 3 month supply, you may NOT receive any more pills, rings, or patches until 3 months has passed.
Wisconsin Medicaid Family Planning Waiver Program
The purpose of this program is to provide contraceptive (birth control) services to eligible women, and related reproductive health care that is part of regular birth control services.
Thanks to the Wisconsin Medicaid Family Planning Waiver Program (FPWP), you might qualify to receive these services at no cost to you.
"Who should consider this program?"
Eligible women wanting contraceptive services.
Overview
Wisconsin women seeking contraception are now able to get confidential family planning health care services and supplies through the Family Planning Waiver Program. The goal of the FPWP is to help women reduce unintended pregnancy and obtain family planning health care services.
"What services are included?"
- Birth control methods, supplies and family planning health care services that include:
- Routine reproductive health exams and tests
- Pregnancy testing and counseling
- Emergency contraception
- Education and information
- If you are using contraception, the cost of care for urinary tract infections, STDs and vaginal infections will be covered (excluding some antibiotics)
"Who can get these services?"
If you answer yes to the following questions there is a good chance you qualify for the Family Planning Waiver Program.
- I am a woman 15-44 years old.
- I am a U.S. citizen living in Wisconsin.
- I do not receive Wisconsin Medicaid, BadgerCare, or Healthy Start.
- I meet the income requirements.*
* Monthly income limits:
one-person household $1,733
two-person household $2,333
three-person household $2,933
four-person household $3,533
"Who can I see?"
Any Medicaid-certified provider can offer the FPWP services. You can receive services from:
- UW Oshkosh Student Health Center
- Planned Parenthood
- Other health care providers may be available in the area. Check with your family provider.
