*For families with more than 8 persons, add $4,180 for each additional person.

 

Income verification

Must be verified 12 months prior to enrollment.  Either 12 months of check stubs prior to the date of application or the calendar year prior to the application, which would be W-2 or tax forms.  We also need to verify child support, unemployment, SSI, W2 benefits or any other income coming into the home. Categorically eligible families where the enrolling child is a foster child or the child is homeless or Public Assistance recipients of SSI, Kinship or W2 are eligible with proper verification as well.
