Admission to Candidacy

Admission to Candidacy is the point in time while completing a graduate program, a student finalizes his or her agreed upon plan of study with the program and the Office of Graduate Studies. The form lists all of the coursework a student has taken or will take to complete his or her degree (similar to an undergraduate STAR Report). All Candidacies require program signatures prior to submitting them to the Office of Graduate Studies.

Candidacy Requirements for All Students

  • Applicants must be in full academic standing and have satisfied all deficiencies specified at the time of admission to the program

  • Applicants must obtain all necessary signatures before submitting the form to the Office of Graduate Studies

  • Applicants must submit three typed or neatly printed copies of the signed candidacy form to the Office of Graduate Studies.  

  • Applicants should discuss any additional program-specific requirements with his or her Advisor or Graduate Program Coordinator

  • If changes to your Candidacy are needed after you've submitted this form, please complete a Program Modification Form to explain those changes.

  • If you are completing both a degree program and a certificate program, than an Application for Admission to Candidacy AND a Certification to Complete Certificate form are required. 

 

Candidacy Forms

There are several different candidacy forms that are required based on your program.  If you are unsure of which form to complete, please ask your Advisor, Graduate Program Coordinator, or contact the Office of Graduate Studies.


Application for Admission to Candidacy

The Application for Admission to Candidacy is our most common candidacy form.  This form is for students who are completing a graduate-degree program, and should be submitted to the Office of Graduate Studies after the completion of 9 graduate degree credits, but before 21 graduate degree credits.  Admission to Candidacy is required before submitting a Research Proposal Form, and no later than one term before the term of graduation.  Please note, your graduate program may have separate, additional candidacy requirements. 

 

Certification to Complete Certificate 

The Certification to Complete Certificate form is for students who are completing a certificate-program.  Submit this form before the completion of half of the certificate credits.  Check with your Advisor or Graduate Program Coordinator for credit requirements before beginning coursework.  

 

Application for Admission to Candidacy for a Graduate Achievement Program (GAP)

The Application for Admission to Candidacy for a Graduate Achievement Program (GAP) is due to the Office of Graduate Studies during the first semester of enrollment.  Approval by the Graduate Program Coordinator and the Director of Graduate Services is required.
