Clinical Paper, Field Project, and Thesis Formatting Process
This information provides an overview of the formatting and completion of clinical paper, field project and thesis manuscripts. The information below is supplemental to the current Office of Graduate Studies Bulletin and Format Policy and Style Manual. For additional information and formatting questions, please contact:

Students must compose a complete first draft of his/her clinical paper, field project or thesis manuscript as described in the Office of Graduate Studies'.
Please call, email, or stop by the Office of Graduate Studies anytime to reach Marci Hoffman with questions.
A complete copy (including the Abstract, Title Page, List of Tables, List of Figures, Table of Contents, etc.) should be sent to Marci at least three weeks prior to the close of the semester or summer session. Emailing a Microsoft Word document or sending a TitanApps (Google Drive) document are preferred; however paper copies delivered or mailed to the Office of Graduate Studies are acceptable.
Clinical Papers and Field Projects: format review can take place anytime a complete manuscript is ready and pending the final review by the advisor. An approved Title Page(s) may be brought at the time of the final review for signature(s). Include as many Title Pages as needed for the number of manuscripts to be bound.
Thesis: preference is that format review take place before an oral defense. If this is not possible, the Title Page must be approved, so the student can obtain advisor and committee signatures following his/her defense. Bring as many approved Title Pages as needed for the amount of manuscripts intended for binding.
Note - The committee may or may not sign at the conclusion of the defense session. However, if they are ready to do so, having Title Pages available may prevent any time conflicts when obtaining these required signatures later on.
After Marci has approved the format, she will send a Format Approval Letter to the student with information about what needs to be completed before the manuscript is bound, circulated through the library and a grade is posted on the student's official record.
On the final day of the 14-week semester or the final day of the 8-week summer session, the following are due to Marci in the Office of Graduate Studies:
One Title Page with original signatures from the advisor, committee chair and/or committee members for each unbound copy intended for binding.
A PDF copy of the complete, approved manuscript (one continuous file, not separated files of each section) emailed to firstname.lastname@example.org. The PDF copy will be added to Polk Library's circulating collection and will also be included in MINDS@UW, a University of Wisconsin System website designed to gather, distribute, and preserve digital materials related to the University of Wisconsin's research and instructional mission.
Note - After the Provost and Vice Chancellor has signed all of the Title Pages, Marci will insert the signed copy to your pdf.
A signed copyright agreement form, which will be provided by Marci with the format approval letter. Additional copies are available in the Office of Graduate Studies.
Unbound, paper copy(s) of the complete, approved manuscript printed on white paper 25% or 100% cotton-rag content, 20lb or 24lb weight substance, standard size (8 1/2 x 11 inches), and does not have an "embossed" texture. Copy This!! in the Reeve Memorial Union keeps this paper in stock and are available for printing.
Number of copies:
The Office of Graduate Studies requires one copy, which will be archived at Polk Library.
The College of Nursing requires one copy of all Nursing clinical papers and theses.
Copies may be required by advisors, committee chairs and/or committee members.
Students may order as many copies they'd like for his/her personal collection or to share with family and friends.
A completed Manuscript Binding Submission Form, which will be provided by Marci with the format approval letter. Additional copies are available in the Office of Graduate Studies.
Ais required if either;
A complete copy (including the Abstract, Title Page, List of Tables, List of Figures, Table of Contents, etc.) could not be sent to Marci at least three weeks prior to the close of the semester or summer session, or
All required materials listed under #5 (above) cannot be turned into the Office of Graduate Studies on the final day of the 14-week semester or the final day of the 8-week summer session.
