Online Forms

• 2012-2013 Additional Funds Received Report Form
  

  • Use this form to report any outside educational resources you anticipate receiving. Examples include, but are not limited to Scholarships, State GI Benefits (not Federal), Veterans Educational Grants, Employee Tuition Reimbursements, etc.

• Consortium Agreement Form
  

• Form to be filled out by a student who wishes to take classes outside of UW-Oshkosh. The classes must be applicable to the UW-Oshkosh degree. **Must talk to a financial aid counselor to determine eligibility**

• Satisfactory Academic Progress (SAP) Appeal
  

• Students shall utilize this form for the purpose of appealing for financial aid eligibility. NOTE: This form is for students determined to be ineligible for financial aid as a result of failing to meet SAP requirements. Students on Financial Aid WARNING cannot appeal.
