Filling out the Application

Please read the following instructions when completing your new license application:

Review the sample application appropriate to the license you are pursuing.
Download the Initial In-State Teaching #PI1602 IS license.
Type in your information.
Print, sign and notarize the application.
Submit the completed application, with the proper remittance to:

"As a mid-career professional seeking to return to teach in the classroom, I was thrilled to learn of UW Oshkosh's EXCEL Center! The Add-on Licensure program was a perfect fit for me!
