Psychiatric/Psychological Disability Criteria
In order to review your request for reasonable accommodation, the following information needs to be submitted from a licensed psychologist, psychiatrist, or licensed clinical social worker:
- A history of the psychiatric/psychological condition.
- A specific, current diagnostic statement as per the DSM-IV which indicates the nature, frequency, and severity of the symptoms upon which the diagnosis was predicated. A diagnosis without an explicit listing of current symptoms is not sufficient. Primary and secondary Axis I and Axis II diagnoses are required.
- Provide the results of tests, interviews, and/or other evaluations used to form the diagnosis.
- Provide the current status and treatment protocol. Please detail prescribed medications and/or treatment schedules that may influence the types of academic accommodations provided.
- A clinical summary that: (1) indicates the substantial limitations to learning posed by the psychiatric/psychological condition; (2) specific recommendations for accommodations that are related to and supported by assessment results; and (3) states how the effects of the psychiatric/psychological condition are mediated by the recommended accommodations.
The information must be provided on the licensed psychologist, psychiatrist, or licensed clinical social worker's letterhead, and must include a license number and signature.
