Criteria for Documenting Medical Disabilities

In order to review your request for reasonable accommodation, we need to receive the following from your medical care provider:

Diagnostic statement

History of problem

Current status and treatment protocol

Prognosis

Test results

Challenges presented by disability

Specific ideas for accommodation

The information must be provided on the physician’s letterhead and signed by your health care provider. The information submitted must be three years old or less.
