are instructions for accommodating a student with a disability. Services
outlined on an Accommodation Recommendation Form (AR) are designed
to promote full classroom access and academic/programmatic opportunity.
Enlargement:
If you need
to enlarge a syllabus, handout, overhead, quiz, examination, or
other class materials for a student with a visual disability, please
send the materials to Mary
Cook of the Dean of Students Office at 125
Dempsey Hall.
In
order to provide optimal services, we need at least three days to
respond. Materials should be modified prior to the actual
class session so that the student can receive or use them at the
same time as other students.
Extended
Time on Exams:
Quizzes or tests
utilizing extended time should be administered in Testing Services.
Faculty members are required to submit the exam, the manila Test
Reservation Card, and Testing Services Cover Sheet to Testing Services
at least 24 hours prior to the scheduled testing time. If
you do not have a Test Reservation Card or a Testing Services Cover
Sheet, please call Carol Smith at (920) 424-0073 or smithc@uwosh.edu
or pick them up at the table outside of Polk Room 4.
Exams should be administered at the same time as the rest of the
class unless extenuating circumstances arise (i.e. extended time
conflicts with consecutive classes). Faculty members and students
should collaboratively determine an appropriate alternate test time.
For Project
Success students, extended time can be offered
in their main office where there are a number of private testing
rooms. In addition, a staff member can pick up the test from you
and return it once it is completed (call Jill Teal at (920) 424-1033).
The Testing Center is also equipped to assist
Project Success students as well as
other students with this accommodation.
Furniture:
Some
physical conditions require the use of podiums, padded chairs, or
other specialized furniture. Facilities Management should have this
furniture in place by the beginning of class if our office is already
aware of the need, and Disability Services will have a sign placed
on it asking that it not to be removed or used by other students.

Notetaker:
Faculty members
are responsible for announcing the need for a notetaker unless the
professor or the student needing the service has approached a qualified
individual with this opportunity.
Naturally we
would ask that you never announce the name of the student needing
the services in class as this would compromise their confidentiality.
Ideally, we are looking for a volunteer that has a cumulative GPA
of 3.0 or higher. The notetaker will be compensated $50 for
the first student and $25 for each additional student that is approved
for notetaking services in the class, and he/she should contact
the Dean of Students Office (125 Dempsey Hall; (920) 424-3100) to
pick up a notetaking pad and make arrangements for payment.
The notetaking pad allows the notetaker to give a copy of their
notes to the student immediately after each class. Notetakers
and students receiving this service also make a commitment to attend
every class barring extenuating circumstances such as illness, family
emergency, etc.
Please
also facilitate a brief introduction of the student with the disability
and the notetaker, and make sure the notetaker has the students
name and your courses name/number before he/she comes to our
office. If you are having difficulty identifying a notetaker, please
contact our office immediately.
Proctor:
Proctors read
the exams to the student and/or write the students responses
to questions. Proctors are instructed not to answer questions for
students nor "embellish" their responses. Mary Cook coordinates
proctoring services. You may contact this person at (920) 424-3126.
Proctoring occurs
in the Testing Center. The student receiving this service
is responsible for notifying Mary Cook when the test needs to be
administered. Should the proctors be unavailable due to her/his
own class commitments, our office will request the faculty member
and the student to reschedule the exam at another time.
Recording
in Class:
The student
should speak with you regarding taping arrangements, and has been
provided a recording agreement form as an OPTION
for you and the student to sign. It addresses
such matters as copywritten material, limits of tape use, destruction
of recordings, etc. The student should not tape any portions of
class when other students are asked not to take notes.
