Criminal Background Check

I paid the background check fee when I applied to the clinical major previously. Do I have to pay the fee again?

Yes.

Do I have to pay the fee for a background check if I have have had one done at my place of employment?

Yes. All students must pay the fee and submit the corresponding paperwork for the background check even if they have had one completed by their place of employment. Please note that additional background checks may be required during the course of your clinical major. Students are responsible for the additional fees to have these background checks completed.

Under the address portion you must put your full address, including street, city, state and zip code. Incomplete or inaccurate information may mean that your application cannot be processed

Do I need to fill out the business name and address of employer?

No

What is considered a "crime" to be reported on this form?

For the College of Nursing, you must report pending charges or convictions related to any laws, including city ordinances, local, state, federal, military or tribal laws.

You DO NOT need to report parking tickets or speeding tickets.

You DO need to report items such as DUI (Driving under influence) and MIP (minor in possession).

What information do I report for a violation?

Provide a typed statement on which you list each crime, when it occurred or the date of the conviction, and the city and state where the court is located. You may be asked to supply additional information including a copies of the judgment of conviction, criminal complaint and police report and other relevant court or police documents. The statement should be attached to the background information disclosure form.

If you check no that you have no violations, and according to the criminal background check it has been found that you replied falsely to this statement, you may be removed from the program for falsifying information. 

After review of the record, you may be asked to provide additional documentation to comply with clinical agency policy (i.e. criminal complaint, judgment of conviction and any other court documents pertinent to clinical agency policy that are necessary). You do not need to provide documentation for these offenses unless you are requested to do so at a later date.
