Demographic Information for Admission Application for Clinical Major to the Traditional Undergraduate Nursing Program

ELIGIBILITY REQUIRES THAT YOU SUBMIT THIS COMPLETED FORM TO THE UNDERGRADUATE PROGRAM OFFICE BY DEADLINE LISTED IN THE COLLEGE OF NURSING ADMISSION POLICY OR AS POSTED ON WEBSITE.

All fields must be completed.  For example, if your permanent and local address are the same enter the exact information in both areas. Local addresses are used to send out acceptance letters.  If a field does not apply to you please indicate with "n/a", i.e. if you do not have a middle initial you would put "n/a" in that field.
  •  It is critical you provide accurate and up-to-date information.  If you do not know your UWO Student ID Number or your UWO Email address it is your responsibility to contact the Registration Office (920) 424-3454 and obtain this information.  You MUST have a valid UWO Email and ID number to complete both Part A and B of the application process.

Please note our SOAR AND SAIL AHEAD programs are a partnerships with UWO College of Nursing but conduct separate interview dates and times and may offer TEAS testing at their site for those applicants only.
