The faculty practicum coordinator for the Fall 2012 semester is Professor Furcy. He will review your application and contact you regarding its status.

Information regarding your Practicum Supervisor

This is the person who has agreed to serve as the supervisor/mentor for your work in the project and will participate in an evaluation of your work at the end of the semester. Be sure you have discussed your project with them and received their approval before you submit this form.
