(This page contains forms that are used by the student) You will need Adobe Acrobat Reader 9 to use the pdf fill-in form. This program is free—download the program by clicking here.  (You only need to download it once and it must be Adobe Reader 9.0 or higher)

Save the on-line form to your desktop (If you complete the form without saving it to your desktop first, the form will be blank when you reopen it.)

To complete the form, Open Acrobat Reader 9.0 through Start - Programs, Once Reader 9.0 is  open, click on file, open, locate your form on the desktop.  You will be able to fill in the fields on the form, and save the form.  When you re-open the form;  the data you had entered will remain.

  This form is used at the beginning of each semester.  One of these must be filled out for EVERY course you are attending through CAPP.  Before submitting the Registration Form, be sure you have followed the steps on "How to Pre-Register for the New School Year" for Pre-registration.

• Request an Official Transcript (Link)  Official Transcripts are not automatically provided.  The student must request an official transcripts.  An UN-official transcript will require logging into Titan web for a copy.
  
  University of Wisconsin Oshkosh Official Transcript is a complete record of a student's enrollment at the University and is maintained by the Registrar’s Office. An Official Transcript includes all undergraduate and graduate courses; this includes high school CAPP courses and Youth Options courses. Partial transcripts are not available. This permanent record is considered confidential between the student and the University. No transcripts are released, except to authorized representatives within the University, without the written permission of the student. An Official Transcript will not be issued unless all obligations to the University have been satisfied. Faxed transcripts are not considered official.
