Form Facts for Faculty & Staff

Below is information to help instructors and students to complete forms for exceptions within the College of Letters and Science. All the forms discussed below are available in the Registration Office. They are also usually available within department offices.

Appeal to Late Add

A Late Add Card can be used to add a class from the end of the first week of the semester to the end of the grace period.

The multi-part form is used AFTER the grace period.

The calendar that outlines the last days to add with and without an instructor's signature as well as without a late add request form can be found on the Registrar's Office Web site.


Important Information to Include on the Form:

  1. The course number (xx-xxx) and the section number (001); 
  2. The five-digit individual class number (from Titan Web); 
  3. Valid address for the student (LOCAL if during the school year);
  4. Professor's signature and printed name;
  5. Grade for the course if the instructor has already submitted grades for the course; and
  6. Student ID number.

Late Adds are almost always approved if the instructor has granted approval.
 

Late Drop

This form is used by students who wish to drop a course after the grace periods that are at the beginning of each semester.

A list of acceptable reasons for requesting late drops are listed both on the back of the student's copy of the form.

Important Information to Include on the Form:

  1. The course number (xx-xxx) and the section number (001); 
  2. The five-digit individual class number (from Titan Web); 
  3. Valid address for the student (LOCAL if during the school year);
  4. Professor's signature and printed name; and
  5. Student ID number.

 

Term Withdrawal

The instructor will probably not be aware of a student's term withdrawal until the process is complete and the student has withdrawn from his/her courses for the term. The student must process this form through the Academic Advising Office (Student Success Center, Suite 202) or the Dean of Students Office (Dempsey 125). A Term Withdrawal means that the student will no longer attend the University for the term.

 

Appeal to Repeat/30-Day Waiver

This Form is usually seen only by the Department Chair. A waiver of the 30 day rule (students are not allowed to sign up to repeat a course until 30 days before the class starts) can be granted under certain circumstances.

Reasons for using the Appeal to Repeat form:

  1. To request permission to register earlier than usually allowed for a repeat course. Generally, students who are registering for a repeat course must wait until 30 days before the class begins. The appeal, if approved, allows for earlier registration.
  2. To request permission to register for the third time in a course.

Important Information to Include on the Form:

  1. Student's valid address (local address if during school year);
  2. Catalog number xx-xxx; and
  3. Department chair's signature (for both classes in the Major and Minor) if the course is being taken for the third time.

Important Information to Include on Form: 

  1. Social Security number and Student ID number;
  2. When using a transferred course that came in as an elective to fill an Area of the major, be sure to give the correct UW Oshkosh number;
  3. Reasons do not need to be elaborated.

 

Independent Studies and Related Readings

Important Information to Include on the Form:

  1. Student ID number and Social Security number;
  2. Independent study Course number OR Related Reading Course number, and department
  3. Session (7W1, 7W2, 14W, 10W, 17W, 3WI, 4W1, 4W2 or 8W)
  4. Term (Fall 2003, Spring 2004, Fall Interim 2003, etc)
  5. Title of the course in 50 characters or less and able to be abbreviated to 30 characters (the title of the course must be different than any existing course titles and must be more specific than "Independent Study"); and
  6. Professor's signature and printed name.
