Student Forms

Two options are given to apply for your field experience.

  1. Select and save the appropriate fillable PDF forms below to your desktop, complete electronically and print the forms. It is highly recommended that you utilize Adobe X, a free program that may be downloaded.

  2. A hard copy of the application may be obtained from N/E 113 (Office of Field Experiences), next to the door of N/E 605 or on the door of the HSL Field Placement Director (N/E 619).

For either option, be sure to attach the preference card to the front of the application and return the completed forms as directed. Miscellaneous Field Placement Forms

 

Hard copies of the Appeal Form and the Out of Area Placement Request  are located in The Office of Field Experience (N/E 113), in the Human Services Leadership Field Placement Office (N/E 619) or outside of N/E 605. Return as directed. Please read forms carefully to accurately complete the information.

HSL Program Forms

  • HSL Program Appeal Form  (Note: This form is for program appeals only. Use the Appeal Form under Miscellaneous Field Placement Forms for appeals related to field placement)

 

A note about placement site changes:

If a change is sought before the semester starts, contact the Field Placement Director to discuss the options available.

If a change is sought after the semester has started, follow these steps:

Contact your University Supervisor to explain the situation and explain your rationale for a change. Arrange a meeting with your University Supervisor to discuss possible remedies.

Ultimately, if the outcome is a placement site change, the University Supervisor will provide a Request for Placement Change form to the student. This form is only accessible to the University Supervisor, not the student.

The student and the University Supervisor complete their particular sections of the form. 

The student must notify the agency supervisor in writing that the placement will be discontinued and thank them for the opportunity. The HSL Field Office and the University Supervisor must be copied on this correspondence.

The student will bring the completed Placement Change Request form to the Field Placement Director (N/E 619) and make an appointment to proceed with a re-assignment.
