Enrollment forms are available at any time at the Children Learning and Care Center office located on the first floor of Lincoln Hall. You can also call (920) 424-0260. Forms should be turned in as early as possible in the semester prior to the session that childcare is needed. Scheduling is done on first come-first serve basis, students having financial aid have first priority. Faculty and staff are accepted after the student priority deadlines. Children from the community are scheduled for preschool sessions as space allows.
To enroll your child you will need the policies and procedures packet. Please read this packet carefully as it contains important information concerning our program purpose, philosophy, health, safety, discipline, transitioning, program, staffing, fees, etc. and necessary forms to complete.
The enrollment form for all children needs to be returned to us promptly to reserve a spot in the first-come, first-serve pool. (On the third page of the student enrollment form, you need to list your schedule for the days and times you are requesting care. Please fill this out completely - including class name, building name, room number, work location, etc). You will be contacted approximately one month prior to the beginning of a session regarding availability of childcare at your specified times.
The following are required forms that need to be returned to the Children's Center:

- Community Enrollment Form
- Electronic Fund Transfer Authorization Form
The child health report for all children needs to be filled out and signed by your child's doctor 30 days prior to enrollment or 90 days after your child's first day of attendance. Children under two need a health report every six months.
You can fill out and sign the Care Immunization Form for all children. It is your responsibility to update it when new immunizations are given to your child. You can retrieve your child's immunization records at the Wisconsin Immunization Registry.
The Child Heath, Health History (all children) and Intake Information Forms (under 2 years only) are to be filled out a day or so before your child's first day of attendance, so as to provide the most current and accurate information. They should be turned in on the first day of attendance.
