To Enroll

Forms should be turned in as early as possible in the semester prior to the session in which you need child care. Scheduling is done on a first-come, first-served basis, with students having financial aid receiving first priority. Faculty/staff are accepted after the student priority deadlines.

To enroll your child you will need the Policies and Procedures Packet. Please read the this packet carefully as it contains important information concerning our program-purpose, philosophy, health, safety, discipline, transitioning, program, staffing, fees, etc.

The Enrollment Form for all children needs to be returned to us promptly to acquire a spot in the first come, first serve pool. (On the second page of the Student Enrollment form, you need to list your schedule for the days and times you are requesting care. Please fill this out completely - including class name, building name, room number, work location, etc). You will be contacted approximately one month prior to the beginning of a session concerning acceptance.

Faculty/Staff Enrollment Form

The Child Health Report for all children needs to be filled out and signed by your child's doctor. This form needs to be returned prior to or on the first day of attendance.

Day Care Immunization Record

You can fill out and sign the Immunization Form for all children. It is your responsibility to update it when new immunizations are given to your child. This form needs to be returned prior to or on the first day of attendance.

The Health History (all children) and Intake Information Forms (under 2 years only) are to be filled out a day or so before your child's first day of attendance, so as to provide the most current and accurate information. They should be turned in on the first day of attendance.

* Open House for the Children's Center for Fall Semesters is always in September on University Orientation Day, the day prior to the first day of Fall Semester classes.

* Open House for the Children's Center for Spring Semesters is always on the last day of Fall Interim, usually the third week of January.

* Parents are welcome to visit the Children's Center at anytime for an orientation. In order to help your child feel safe, secure and welcome in our program we encourage you to come and spend time at the Children's Center with your child during the session prior to your child's starting date.
