Returning Athlete Information

The Athletic Training and Medical Staff welcome you back for the upcoming school year. Returning athletes must complete the following forms prior to any participation (practice or event) in intercollegiate athletes. You will also need to access our medical website called "ATS" and update all your information and make sure it is correct. ALSO if a student-athlete is using a banned stimulant drug to treat Attention Deficit Hyperactivity Disorder (ADHD) or Attention Deficit Disorder (ADD), you will need to fill out the following ADHD Medical Exception Drug Form and your physician must sign this form and attach supporting documentation. A sample ADHD Evaluation Form is also provided below. You may return these documents to us via fax (920) 424-1068 or by mailing them to the following address:

* Annual physical updates are offered on campus twice per year. (In October for winter and spring sport athletes and in April for fall sport athletes) Dates and times will be posted on web site. If you are unable to attend these sessions you may have your update completed at the Student Health Center or by your personal physician. This must be physically handed in.

Insurance Form (Opens up as a .PDF file)

Check to make sure all is current on ATS medical website. If your insurance has changed we need you to update the changes on the ATS medical website and also physically hand in the health insurance form. A photocopy of both the front and back of the health insurance card is also to be handed in if your insurance has changed.
