Returning Athlete Information
The Athletic Training and Medical Staff welcome you back for the upcoming school year. Returning athletes must complete the following forms prior to participation (practice or event) in intercollegiate athletes. You may return these documents to us via fax (920) 424-1068 or by mailing them to the following address:

* Annual physical updates are offered on campus twice per year. (In October for winter and spring sport athletes and in April for fall sport athletes) Dates and times will be posted on web site. If you are unable to attend these sessions you may have your update completed at the Student Health Center or by your personal physician.
