Sickle Cell Trait Verification

Any new athlete must have verification of their sickle cell trait test, which is attached to the physical form, starting in August of 2013. You are tested at birth for this trait. Your physician may verify this with the hospital at which you were born and then sign the form. If you do not know your status, you may opt to have a simple blood test conducted by your physician to determine your status or may ask for a waiver, in which you would sign when coming to UW Oshkosh. Those that choose the waiver, will need to comply with additional education once coming to UW Oshkosh.

 

ADHD/ ADD DOCUMENTATION

Beginning in the fall of 2012, the NCAA is requiring all student-athletes who have been diagnosed with Attention Deficit Hyperactivity Disorder (ADHD), Attention Deficit Disorder (ADD) and/ or like conditions, and are currently prescribed banned stimulant drugs (ex. Ritalin, Stattera, adderall, Concerta, etc.) for treatment, to have proper documentation handed in prior to any athletic participation. Effective August, 2012, the NCAA now requires documentation of a comprehensive clinical evaluation to support treatment with NCAA banned stimulants. The following will need to be on file with UW Oshkosh Sports Medicine prior to participation: 1) ADHD Medical Exception Drug Form and 2) Supporting clinical evaluation notes from your physician who diagnosed you with ADHD or ADD. These forms along with a sample evaluation form for your physician can be found by clicking on the Athlete Information/ Forms link to the left and under both New UW Oshkosh Athlete and Returning UW Oshkosh Athlete. We recommend starting this process early, in order to have all forms complete prior to arriving at UW Oshkosh in the fall.

 

NEW UW OSHKOSH ATHLETE INFORMATION

If you are an incoming athlete and new to UW Oshkosh, the following information must be submitted before the athlete is allowed to compete in their sport: 1) Pre-participation questionnaire 2) Physical exam form 3) Health insurance form and 4) Photocopy of both the front and back of the health insurance card. ALSO, NEW IN THE FALL OF 2012, if a student-athlete is using a banned stimulant drug to treat Attention Deficit Hyperactivity Disorder (ADHD) or Attention Deficit Disorder (ADD), we need you to fill out the ADHD Medical Exception Drug Form and your physician must sign this form and attach supporting documentation. RETURNING UW OSHKOSH ATHLETE INFORMATION

Returning athletes will need to submit the following information before being allowed to compete in your sport: 1) Physical update form completed by physician 2) Health insurance form if it has changed in anyway from the previous year and 3) Photocopy of both the front and back the health insurance card if your insurance has changed. ALSO, NEW IN THE FALL OF 2012, if a student-athlete is using a banned stimulant drug to treat Attention Deficit Hyperactivity Disorder (ADHD) or Attention Deficit Disorder (ADD), we need you to fill out the ADHD Medical Exception Drug Form and your physician must sign this form and attach supporting documentation. The physical update form should be completed by the athlete and brought to a physician for completion. Physical updates can be performed by making an appointment at the Student Health Center, 920-424-2424, or by your family physician.
