NEW UW OSHKOSH ATHLETE INFORMATION
If you are an incoming athlete and new to UW Oshkosh, the following information must be submitted before the athlete is allowed to compete in their sport: 1) Medical history questionnaire 2) Physical exam form 3) Health insurance form and 4) Photocopy of both the front and back of the health insurance card. RETURNING UW OSHKOSH ATHLETE INFORMATION
If you are a returning athlete who competes in a Fall sport and did not participate in physical updates last April, you will need to submit the following information before being allowed to compete in your sport: 1) Physical update form completed by physician 2) Health insurance form if it has changed in anyway from the previous year and 3) Photocopy of both the front and back the health insurance card if your insurance has changed. You may download a copy of these forms by clicking on UW Oshkosh Athlete link and then Returning UW Oshkosh Athletes. The physical update form should be completed by the athlete and brought to a physician for completion. Physical updates can be performed by making an appointment at the Student Health Center, 920-424-2424, or by your family physician.
