Welcome to UW Oshkosh! The Athletic Training and Medical Staff look forward to serving you and any medical needs that you may have in the future. Please do not hesitate to contact us if you have any medical problem. We are here to help and provide quality health care to all Titan Athletes.
All athletes NEW to UW-Oshkosh or returning after a leave from competition must complete a 1) medical history questionnaire 2) physical exam and 3) insurance form prior to participation (practice or event) in intercollegiate athletes. The medical history questionnaire is to be completed on our medical website called ATS and this information is below. ALSO, NEW IN THE FALL OF 2012, if a student-athlete is using a banned stimulant drug to treat Attention Deficit Hyperactivity Disorder (ADHD) or Attention Deficit Disorder (ADD), we need you to fill out the following ADHD Medical Exception Drug Form and your physician must sign this form and attach supporting documentation. A sample ADHD Evaluation Form is also provided below. You may return these documents to us via fax (920) 424-1068 or by mailing them to the following address:

Medical History Questionnaire (To be completed on ATS medical website)
In order to complete you will need to access the ATS medical website at https://www.atsusers.com/Default.asp and will need a username and password. This information can be obtained by contacting your sports athletic trainer via phone or email. The complete list of sports athletic trainers can be found by clicking here.
Physical Form (Opens up as .PDF file)
To be completed by physician and physically handed in.
Insurance Form (Opens up as a .PDF file)
To be completed on ATS medical website and also physically handed in. A photocopy of both the front and back of the health insurance card is also to be handed in.
