Student Employee Mini-Evaluation

The supervisor classifies the student for each item as:
doing very well; acceptable; or could improve.

1.  Attendance – as scheduled, on time
2.  Appearance – professional, appropriate for job
3.  Attitude – positive towards customers, coworkers, show enthusiasm
4.  Customer Service – acts to meet/exceed customer needs
5.  Job Performance – performs all aspects of job, completes tasks accurately and in timely manner
6.  Communication – appropriate, effective communicator – both orally and written, as needcd
7.  Relationship Building – established rapport with customers, is an effective team member
8.  Relationship Building – relates to many types of people including those with different backgrounds/values
9.   Adaptability – changes behavior in order to positively cope with working situations
10.  Stress Tolerance – deals effectively with pressure, deadlines, conflicts, ambiguous situations
11. Initiative – assumes extra/new job responsibilities, sees necessary tasks and completes them without being told, works well without supervision
12. Decision Making – thinks through situations and arrives at acceptable solutions
